MODERNA COVID-19 VACCINE CLINIC
18 years of age and older

Nutley Residents, aged 18 years and older, are eligible to receive
the 1st and 2nd doses of the Moderna Covid-19 Vaccine.

Residents that fit the criteria below are eligible for
the 3rd Moderna COVID-19 Vaccine dose, of the three-dose series.

Once you click on the SUBMIT button, your form will be forwarded to
the Nutley Health Department.

The date and time of your appointment will be emailed to you with instructions.

Who Needs an Additional COVID-19 Vaccine?

Currently, CDC is recommending that moderately to severely immunocompromised people receive an additional dose. This includes people who have:

• Been receiving active cancer treatment for tumors or cancers of the blood
• Received an organ transplant and are taking medicine to suppress the immune system
• Received a stem cell transplant within the last 2 years or are taking medicine to suppress the immune system
• Moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome)
• Advanced or untreated HIV infection
• Active treatment with high-dose corticosteroids or other drugs that may suppress your immune response

Residents should talk to their healthcare provider about their medical condition, and
whether getting an additional dose is appropriate for them.
